ALL BREED CHAMPIONSHIP SHOW – ENTRY FORM

INSTRUCTIONS:

The name of the dog and the details as recorded with the Trinidad & Tobago Kennel Club must be given on this entry form. If an error is made the dog may be disqualified by the show committee.
All dogs must be REGISTERED at the Trinidad & Tobago Kennel Club. If a registered dog has changed ownership the TRANSFER must be registered before the date of the SHOW.
ON NO ACCOUNT WILL ENTRIES BE ACCEPTED WITHOUT FEES.
A PUPPY under the age of six (6) months cannot be exhibited.

“A” Puppy For dogs not less than six (6) months and not exceeding twelve (12) Months of age on the day of the show.
“B” Junior For dogs not less than twelve (12) months and not exceeding Eighteen (18) months of age on the day of the show.
“C” Limit For dogs that have not won three (3) Challenge Certificates under Three (3) different judges.
“D” Open All dogs.

DECLARATION: I undertake to abide by the Rules and Regulations of the Trinidad & Tobago Kennel Club and of this Show and I declare that the dog (s) entered have not suffered from or been knowingly exposed to the risk of distemper or any contagious disease including any reaction to immunization during the six (6) weeks prior to exhibition and I will not show the dog(s) if they incur such risk between now and the day of the show or if they have been immunized within fourteen (14) days prior to the show. I declare that to the best of my knowledge the dog(s) are not liable to disqualification under the Trinidad & Tobago Kennel Club Regulations.
